Community-based services take place outside of traditional facilities. These are programs provided by or at community centers and adult day cares. 

Community-based services emphasize helping community members in an alternative way. One of the ways they do this is by connecting services directly to individuals’ homes. The goal is to make services accessible and help extend the time people get to live at home while receiving care.   

Community-based services focus on building social connections and trust within the community. Common services or programs may include:
