A letter of decline notifies an insurance applicant of the company's decision not to insure them. This is a formal denial letter that explains what disqualified the individual from coverage. 

These letters include a detailed description of why the company chose not to insure the applicant. This usually includes the company's health standards and references parts of the applicant's medical history or application that were not eligible. 

They may also include contact or appeals information. This allows applicants who feel the insurance company made an incorrect decision to appeal the decision.
