Summit Home Care & Hospice provides in-home medical care and hospice services to families across the Columbus area from its office on Yard Street in Grandview Heights, Ohio. Its care team brings skilled nursing; physical, occupational and speech therapy; home health aide support; and medical social work directly to patients' homes, helping older adults recover after a hospital stay or surgery, manage chronic conditions and safely age in place. For patients with a serious illness, Summit's hospice program focuses on comfort, pain and symptom management, and support for the whole family. Summit also serves communities in Cincinnati, Indianapolis and Nashville.

Freedom Caregivers opened its doors in 2013, responding to a critical need for local home health care services for the elderly population. Its founders were one physical therapist and one nurse who envisioned an organization that could offer the quality of care they would expect for their own elderly family members.
