Puerto Rico offers a range of long-term care services including home health care, assisted living, and skilled nursing facilities. There are approximately 922 licensed long-term care homes on the island, though only 9 are Medicare-certified skilled nursing facilities with a combined 299 beds.
The territory does not participate in the Long-Term Care Partnership Program.
Medicaid
Puerto Rico’s Medicaid program operates under a capped federal allotment rather than the open-ended matching funds available to states. As a result, nursing facility care is not a covered Medicaid benefit in Puerto Rico. The cost of long-term care falls largely on the elderly and their families — a significant difference from the 50 states, where Medicaid is the primary payer for nursing home care.
Puerto Rico is working to expand its Medicaid-covered services, with nursing home coverage expected as part of programmatic improvements targeted for 2027 and beyond.

Tax Incentives
Puerto Rico has its own tax system separate from federal income tax. Most Puerto Rico residents do not file federal income tax returns, so the federal tax deduction for Long-Term Care Insurance premiums does not apply to most residents. Residents with U.S.-source income who file federal returns may still qualify for federal tax benefits. Puerto Rico does not currently offer a territory-level tax incentive for LTC insurance premiums.
